Online Access Registration
To register for online access to SystemOnline, please fill out the form below, and bring it to Cambridge Avenue Surgery along with two forms of identification (preferably one which has a photograph such as a driving licence).
You will then receive a user name and password that will enable you to visit SystemOnline.
With this you can:
- Book appointments
- Requesting repeat prescriptions
- Viewing Summary Care Record
- Accessing my Detailed Coded Record (this may take 28 days to complete)
